Start a discharge session

Pick where the patient is going and what they need. You will get services they can actually reach, and a sheet to print for them. No patient name or record is ever entered here.

1. Where is the patient going

Shelters and safe havens are public facilities on a finite, curated list, so they are selected rather than typed. That removes the geocoder entirely, and with it the only step in the flow that would have sent an address to a third party.

2. How far can they travel

3. What language do they speak

Selecting a language does two separate things. It grades every result by how the site actually delivers that language, and it decides which versions of the printed guide get produced. Languages marked print pending can still be used for matching, but the guide will not print in them until a speaker has reviewed the phrase bank.

4. What could stop them getting in

5. What do they need

Health care

Food, hygiene and clothing

Benefits, housing and legal

Mail, transit and storage

This list is the standing referral set for the session. Filters on the next screen narrow it but never widen it, so nobody hands out a resource the discharge plan did not ask for. Leave it empty to browse everything.
